The summer heat has finally settled down, and the weather is becoming more comfortable!<\/p>\n

This blog is for those who have been bitten by insects during the summer, leading to hardened skin that won\u2019t heal and persistent itching.<\/p>\n

At the end, I will also introduce the laser treatment we offer at our clinic.<\/p>\n

Most cases of hardened skin and persistent itching are caused by black flies (buffalo gnats).<\/h3>\n

First, black flies inhabit mountain streams across Japan and feed on blood during their breeding season from spring to late summer.<\/p>\n

During their egg-laying periods in spring and summer, female black flies attack humans to obtain nutrients. This often happens in outdoor activities such as camping, golfing, fishing, and farming.<\/p>\n

The tricky part is that black flies are small and make little to no buzzing sound.<\/p>\n

As a result, they can approach unnoticed, and people often don\u2019t realize they\u2019ve been bitten.<\/p>\n

Additionally, general insect repellent sprays are not very effective against them (tears).<\/p>\n

Symptoms of a Black Fly Bite (Acute Phase)<\/h3>\n

If you see a swollen bite area with a red bleeding spot in the center, it is almost certainly a black fly bite.<\/p>\n

Immediately after being bitten, a small part of the skin is damaged, and blood or fluid may ooze out.<\/p>\n

Within half a day to the next day, intense itching and redness will develop.<\/p>\n

People are often advised to “resist scratching with iron willpower!” but in most cases, they end up scratching.<\/p>\n

Treatment:<\/strong> Topical steroids, oral antihistamines, and oral steroids.<\/p>\n

Symptoms of a Black Fly Bite (Chronic Phase)<\/h3>\n

Scratching stimulates the nerves responsible for transmitting itchiness, making the itch even worse. Continuous scratching causes the skin to harden and thicken.<\/p>\n

As a result, the skin around the bite becomes thickened and raised, and even with treatment, the symptoms do not improve, and the itching persists for a long time.<\/p>\n

This condition is called “Nodular Prurigo.”<\/strong><\/p>\n

In severe cases, symptoms may last for years, significantly impacting sleep, daily life, and mental health.<\/p>\n

Laser Treatment for Nodular Prurigo<\/h3>\n

Removing the center of the nodules with a laser can help relieve the itching.<\/p>\n

Common treatments for nodular prurigo include topical steroids, oral steroids, and occlusive dressing therapy (ODT).<\/p>\n

Occlusive dressing therapy<\/strong> involves applying an ointment to the affected area and covering it with a film such as plastic wrap.<\/p>\n

If these standard treatments successfully reduce itching, nodular prurigo will gradually improve.<\/p>\n

When Itching Persists and Becomes Difficult to Treat<\/h3>\n

However, nodular prurigo is often difficult to treat<\/strong>, and in some cases, itching continues even with medical intervention.<\/p>\n

If the itching persists, continuous scratching causes the nodules to enlarge, dry out, become inflamed, and intensify the itch even more.<\/p>\n

Once this negative cycle<\/strong> begins, treatment becomes very challenging and time-consuming.<\/p>\n

To break this cycle, our clinic uses a laser to remove the center of the nodule.<\/strong><\/p>\n

The procedure takes about five minutes<\/strong> after local anesthesia.<\/p>\n

Afterward, the itching subsides significantly, and the hardened skin gradually softens and heals.<\/p>\n

What Happens After Treatment?<\/h3>\n

The treated area will leave a scar similar to a regular wound or burn mark, as shown in the image below. However, after about six months<\/strong>, the scar blends with the surrounding skin and becomes much less noticeable.<\/p>\n
